I understand that dance and related activities involve risk of injury, I agree I will not hold The Sjaan Dance Academy or its employees, officers liable for any injuries/damages incurred by me or any of my family members while participating or visiting the facility and participating in the classes.

I give permission for photographs/videos will be taken during the lessons which will be used for publicity purposes and advertising purposes.
